  • You will need to follow these simple steps to allow the school to assist your needs:
 
1.   The first point of contact if your child has severe food allergens is your school’s nurse.
 
2.   You will receive federal forms that must be completed by you and your child’s doctor.  In these papers will be a list of products that you and your doctor will need to identify as appropriate or not appropriate to serve to your child for meals.
 
3.   Return the forms to your child’s school nurse.  They will share this information with the school cafeteria manager.  The information is entered into your school’s cafeteria POS system.  In extreme cases, we may ask a meeting be set up with you, the school’s nurse and the Food Service Department.
 
4.   A menu is then created to meet the National School Lunch requirements, as well as the child’s allergy. 
 
5.   When your child goes through the lunch line the POS system register displays a warning alerting the cashier of the child's allergy.
 
All students with allergies are required to complete the “Physician’s Statement for Students with Special Dietary Needs” Form. 
The form MUST be returned to your school nurse. 
 The school nurse will ensure the Food Service Department receives a copy.
