KINDERGARTEN REGISTRATION will begin

Monday, April 12, 2010

9:00 a.m. to 2:30 p.m. 
To enter kindergarten in the fall, a child must be 5 years old on or before Sept. 30.  All new student registrations will be conducted online at www.spotsylvania.k12.va.us.  Pre-registration forms will be available online starting April 1st.  Please go online and pre-register your child as soon as online registration is available.  Once pre-registration is complete, you must complete the registration process at your child's school site.

Documents Needed:  Original Birth Certificate; Social Security Card;

2 Proofs of Residency (1st must be a lease, mortgage or contract and

the 2nd can be a current utility bill).  A physical and shot record is

required before any student can enter school.

************NEW IMMUNIZATION REQUIREMENTS*************

for the 2010-2011 school year

A second dose of Varicella vaccine is required before entering kindergarten.

A second dose of mumps vaccine is required before entering kindergarten.

Four (4) doses of polio vaccine are required, with one (1) dose being administered after the 4th birthday, for all new children enrolling in school (K-12)

Four (4) doses of DTaP vaccine are required, with one (1) dose being administered after the 4th birthday, for all new children enrolling in school (K-12)

Parents are responsible for providing documentation that all required immunizations are up-to-date, completed, or that a medical or religious exemption has been obtained.

The information on page 3 of the MCH 213 F physical form is not correct and these are the new state mandated school entry requirements effective 

March 3, 2010.
